Purpose of Position:
To deliver an effective professional social work social work service which is designed to respond to the individual needs of clients and their family/whanau. To work collaboratively with our colleagues from other disciplines within the multi-disciplinary team and liaise with and advocate on behalf of clients with related agencies to provide a comprehensive service to all clients.
Key Accountabilities:
1. Clinical / Case Management
- As part of the multi-disciplinary team in the role of social worker you will contribute to the assessment, planning and implementation of services with clients and their families
- You will maintain a manageable caseload as a keyworker.
2. Team work
Participates as an active member of the multi-disciplinary team, contributing a psycho-social perspective utilising best practice standards.
You will work alongside colleagues to promote the optimal outcome for client
You will respond to social work specific referrals from colleagues.
3. Professional Development
You will take responsibility for your ongoing professional development and ensure your supervision needs are met.
4. Quality Improvement
You will participate in service development and quality of service activities to enhance outcomes for clients and improve co-ordination of care across services.
3. Cultural Sensitivity
- You will practice in a way that acknowledges and respects difference
- You will demonstrate a commitment to practising social work in accordance with the Bicultural Code of Practice and an understanding of the principals of Te Tirioti O Waitangi
Essential:
- Degree in Social Work
- Registered social worker
Desired:
- Post-graduate qualification in Social Work or Mental Health
- Experience in MH / working with older people
