POSITION DETAILS
TITLE: Primary Care Nurse, Community Mental Health Centre
Reports To: Clinical Team Leader, Taylor Centre CMHC.
PRIMARY FUNCTION:
The primary care nurse provides primary nursing care support, patient interventions and treatment within the community mental health setting. The primary care nurse will use their clinical skills and expertise to link services users to other services and care provides including primary health care and ngo’s. The primary care nurse will work alongside and in active collaboration with the cmhc multi-disciplinary team, primary care providers and cmhs primary mental health nurse specialists and nurse practitioners.
